The win reignited the Blues Champions League qualification hopes.
Alex Iwobi fired Fulham ahead from the edge of the box during the first half.
Substitute Tyrique George levelled in the 83rd minute, just five minutes after replacing striker Nicolas Jackson.Neto fired home the winning goal in the 93rd minute.
The win lifted Chelsea into fifth, a point behind Manchester City.
